I recently discovered what seems to be a systemic problem with the SSA online application for spousal benefits and would like to bring this to the attention of an appropriate person at SSA. My wife applied online but was awarded retirement rather than spousal benefits. We have now been told by three different SSA workers that this is a common problem with online spousal benefit applications. All three also said that my wife must file a withdrawal of application form, wait for SSA to approve the withdrawal, and then apply in person for spousal benefits.

I don't have any experience with Social Security's online application process since only people who actually wish to file for benefits are permitted to view the online applications. So, I wasn't aware of this problem, nor is there any way for me to determine the likely cause(s). I also have no idea whom within SSA is in charge of the online application process. You might try contacting a supervisor or manager at your nearest office, and ask them to pass along your concerns to the appropriate individuals.
It sounds like your were given correct information by the Social Security employees with whom you spoke, though, assuming that your wife is full retirement age and permitted to file a restricted claim for spousal benefits only. If her online claim was mistakenly submitted as a claim for retirement benefits, she would need to formally withdraw that claim and then apply for spousal benefits.
